To effectively apply precision medicine, it is imperative to extract and interpret patient signs and symptoms, recorded as free text in electronic health records. The computational representation of extracted signs and symptoms is facilitated by their mapping to the corresponding entries of signs and symptoms in an ontology. Identifying signs and symptoms within free-form text is a slow and arduous process. Prior investigations have shown that the concordance between raters in extracting clinical concepts is generally poor. We assessed the agreement between different annotators in tagging neurological concepts present in clinical notes extracted from electronic health records. Having been trained on the annotation method, the annotation program, and the supplementary neuro-ontology, three raters annotated fifteen clinical notes in three iterations. The three annotators showed a high degree of agreement in classifying text spans and assigning category labels. The convolutional neural network machine annotator showed a strong alignment with human annotators' classifications, but this concordance remained below the superior level of agreement observed among the human annotators themselves. We determine that high levels of concordance between human annotators can be achieved with the proper training and annotation resources. Ultimately, greater training data sets, coupled with enhanced neural networks and natural language processing methodologies, are predicted to allow machine annotators to automate the high-volume extraction of clinical concepts, with a level of agreement closely matching that of human annotators.
